Toyota Sienna Hybrid, only available in Japan since 1997. (The American market wouldn't allow it to be sold in the U.S. because it would compete with the already successful Toyota Sienna not hybrid.) Japan's Sienna Hybrid gets 40mpg. It seats 7.
According to hybridcenter.org, they have announced it to be released in the U.S. in 2009, although not as good a model as is available in Japan. However, they keep saying they will release it and then it keeps getting postponed. It would be the first hybrid minivan in the U.S. It is said it will only get less than 35mpg and have a smaller drivetrain than the model available in Japan.
